The Tennessee Division of Consumer Affairs accepts complaints from residents of Tennessee, and from people who have had problems with a business located in the state of Tennessee.
Thank you for reporting this problem to the Tennessee Division of Consumer Affairs. Although this office does not represent individual consumers, it will attempt to mediate your complaint.

To file your complaint, please fill out all sections of the form below. Provide as much information as possible. Select the Submit button at the end to forward the information to us.

All fields marked with an asterisk (*) are required. If you have supporting documents, receipts, canceled checks or a contract you must go back and use the printable complaint form and either fax or mail that form with the required documentation.
