(Options of legal verification accepted by the Office of Teacher Licensing are: a photocopy of the notarized or certified marriage license, or divorce decree or the court order that has generated the legal change of name. The Social Security Card is not an acceptable form of legal verification of name change). Only (1) copy can be requested.

If you have an expired license, no changes can be made to your license. The Office of Teacher Licensing can not provide you with a copy of an expired license.
