Type and submit the application form. It is in your best interest to apply promptly. Acceptance for academy participation is on a first-come, first-served basis, and space is limited. You may apply for only one event per form. Duplicate the form as needed for each additional event.
Each academy has an identifying 5-digit number that must be entered on the application form. Make certain the 5-digit number matches your application choice.
Provide the name you want used on correspondence and nametags. Be sure to enter your social security number. Applications without a social security number will not be processed. Applicants must supply an evening phone number that may be used if it becomes necessary to reach individuals when school is not in session.
Applying for an academy does not indicate acceptance as a participant. Applicants will be notified of their status in each academy requested within four (4) weeks of application. Contact Debbie Hines (firstname.lastname@example.org) to make inquiries regarding the applications status. Upon acceptance to an academy, each participant will receive an academy "fact sheet" that outlines academy details, including venue, driving directions, and time.
