Register for the Blind

T.C.A. 49-10-804 mandates that the Department of Human Services maintain a register of citizens who are legally blind and living in Tennessee. Ophthalmologists, optometrists, and other medical professionals are required by this law to report to the register the names and pertinent facts of any person who is examined and diagnosed to be legally blind. All information provided to the register is confidential and is used only for purposes specifically related to the administration of the program. A referral form for this purpose can be found in the link that follows. The referral form includes instructions for submission and agency contact information.
