System Transformation
In collaboration with the Department of Intellectual and Developmental Disabilities (DIDD) and other stakeholders, TennCare is working to align long-term services and supports (LTSS) programs with a mission to promote person-centered principles, supported decision-making, inclusion, and self-advocacy within our service delivery system. We call this work “System Transformation.” TennCare is working alongside many stakeholders including DIDD, other state agencies, health plans, providers, advocacy groups, people supported through LTSS programs, and family members to advance System Transformation initiatives, including IDD Program Integration. Members from these stakeholder groups participate in the Statewide Transformation Leadership Group (STLG) – an advisory group that guides System Transformation work and other positive change. Together, the STLG created “infographics” that illustrate the core values of System Transformation. We believe that our System Transformation efforts will help to achieve a broader culture transformation where people with disabilities are better supported to enjoy the rights, valued roles, and quality of life that other citizens are afforded.
